3. Professional Licenses and Certifications

   • PRC License and Certificate

   • Current PRC ID (with Number and Expiry Date)

4. Training and Accreditation

   • Residency Training Certificate

   • Sub-Specialty Training Certificate

   • Specialty Board Certificate

   • Sub-Specialty Board Certificate

5. Membership and Affiliation

   • PMA (Philippine Medical Association) ID Number

   • PhilHealth Accreditation Details:

   • ID Number

   • PIN

6. Other Identification Numbers

   • TIN (Tax Identification Number) with TIN ID​​
